Support for creation of input.csv - Implementation - with custom functionality from our cohort creation scripts

Currently the user needs to fill the input.csv themselves.
We should support them in this. They should be able to specify some selection criteria and then the tool provides the input.csv for them.
This should happen in a seperate script and with a seperate command.

Here we provide functions to select records based on:

• available channels in records and minimum duration of those records (based on mimic_waveform_explore_helper.py get_waveform_records_with_signals_and_minimum_recording_duration)
• whether patient had a specific procedure before the icu stay via cpt_code (cohort_creation_cardoac_surgery.py and cohort_creation_mediastinum_diaphragm_respiratory_surgery.py)
• whether patient is surely on mechanical ventilation (or surely not) during duration (cohort_creation_ventilation.py)
• whether patient surely receives medication during duration by itemid of mimiciii.inputevents_cv (we need to extend this to support mimiciii.inputevents_mv as well). (cohort_creation_vasopressors.py)

Please implement this.
