fix(web): finish Codex sign-in when the tab becomes visible again

The sign-in composable polled the login status on window focus only. A
tab that was hidden and shown again without a focus event (mobile tab
switch, some desktop window managers) kept waiting for the interval.
Poll on visibilitychange too, when the document is visible, and remove
the listener on unmount. Rebuild the shipped web bundle from source.
