Add new field in patient login screen

For now, we should call the New field "functional score".

The field will not be mandatory today. But, at some point, patients will include a validated test score that can be correlated to the CTSIB test.

I'd love to have the field on there and talk through this with prospects as a selling point when doing the demo.
