LLM claims no new image is attached

LLM does not deal well with jp q -a ... mid-conversation. It is willfully ignoring that I definitely sent an updated image with a different name. Says it only sees original image, etc.

Correct. I have noticed the same. It is because we update attachment data in older turns instead of re-attaching. This is a failed experiment and I will change the implementation.

Opus 4.8 now advertizes the ability to update metadata and permissions without invalidating cache. Maybe give this a second chance or develop a sophisticated integration test that can separately spot the context failures and the cache invalidation, as other providers will see a lot of differences in this feature's stability.
