Turn normalized source changes into a concise chapter impact set.

Acceptance criteria

• Unchanged sources produce no change set.
• Changed headings and paragraphs produce a readable summary.
• Only directly mapped chapters are marked affected.
• Failed refreshes leave the published snapshot untouched.
• Partial rebuild behavior is deterministic and tested.
