feat: add Command Code quota cards

Summary

• Add Command Code as a live Resetwatch provider via COMMAND_CODE_API_KEY.
• Read account identity, subscription, credit balance, 5-hour/weekly windows, and billing-period usage from the alpha endpoints used by the reference provider.
• Keep optional endpoint failures partial and add hermetic probe coverage for parsing, auth, URLs, and account validation.
• Update the plugin UI copy and README, and bump the plugin version to 0.2.11.
