[codex] Align AppLens Tune consent wording

Summary

• replace stale Store V1/read-only Tune wording with the current AppLens/AppLens-Tune split
• add action-log output to Markdown and HTML reports
• update readiness/report tests so generated output does not reintroduce the old read-only V1 promise

Notes

• Preserves accurate scan-layer language: AppLens scans locally and is read-only by default.
• Positions AppLens-Tune as selected, consent-based actions with local action logging.
• Leaves unrelated local leftovers unstaged/stashed outside this PR.
