Add Task ISA (detect-only) and Capability Doctor - #114

Phase 1 is detect-only: claim closure never mutates consensus, and stock phase artifacts cannot invent closed. Capability Doctor reports live/broken/ declined/unconfigured without replacing CI. Formal pre-commit review gating is deferred to a High-tier follow-up. Co-authored-by: Cursor <cursoragent@cursor.com>
