feat(remote-dsh): bridge DSH 0.1.1-rc.2 capabilities

Summary

• Upgrade the Orbis DSH plugin to the 0.1.1-rc.2 SDK.
• Add capability-gated image upload/download, prompt reference completion, Ask User responses, plan mode, and read-only subagent listings.
• Project DSH questions, goal/todo state, run activity, presence, image references, and interrupted messages into the canonical Orbis v2 model.
• Expand protocol, adapter, host, plugin, and real-profile smoke coverage.
