feat: ship the internal Knowledgebase operator dashboard

Summary

• remove the obsolete standalone landing package and keep the dashboard as the sole Pages surface
• protect the dashboard with verified Cloudflare Access identity and a same-origin Pages Function proxy
• keep the Worker service credential server-side
• add Data inspection for files, chunks, jobs, entities, relationships, failures, and provenance
• expand Query History with filtering, pagination, citations, export, and quality drilldown
• add a tenant-scoped Worker chunk-list route and regression coverage
• document and archive the OpenSpec change and production access runbook

Validation

• Worker check: 33 files, 355 tests passed
• Dashboard typecheck and lint
• Dashboard production build
• Pages Functions build
• Documentation checks
• Strict OpenSpec validation
• Desktop and narrow responsive browser walkthroughs
• git diff check

Release

Production release is intentionally Worker-first, then Pages after Cloudflare Access and Pages runtime bindings are configured.
