ci-alert: match title-or-label and poll before opening a second issue - #13

Tested in claude-reads-hn on 2026-09-02: the REST issue list lags a write by a few seconds, unevenly, so two opens close together made two issues even with the label filter. Match on title OR label over the plain open list and poll up to 15 s before creating. Same script as the other two repos; live-tested here (open, open, close -> one issue, one comment, closed; test issue deleted). Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>

opens close together made two issues, label filter or not. Match on title OR label over the plain open list and poll up to 15 s before creating. Live-tested on this repo:open,open,closegives one issue, one comment, closed (test issue deleted). Does not touch anything in #9.
